General Release of Liability

I acknowledge that volleyball training involves physical activity, which carries risks of injury, disability, or death. I understand that:

• Participation in volleyball training, lessons, clinics, and drills may lead to injury even when safety measures are followed.

• I am participating voluntarily and assume all risks related to involvement in CourtCraft Volleyball activities, whether known or unknown.  

On behalf of myself and/or the minor participant named above, I hereby waive, release, and discharge CourtCraft Volleyball, its owners, coaches, employees, contractors, agents, volunteers, and affiliates (collectively “Released Parties”) from any and all liability, claims, demands, injuries, damages, costs, or causes of action arising out of participation in training events or use of training facilities, including claims resulting from the negligence of the Released Parties to the fullest extent permitted by law. 

Photo/Video Consent (optional)

I consent to the use of photographs or videos taken during training sessions for promotional, marketing, or educational purposes without compensation.

Code of Conduct

The athlete & parent(s) agrees to respect others and maintain appropriate behavior during sessions. Courtcraft Volleyball reserves the right to remove any participant for unsafe or disruptive behavior.
